Vinfen in Boston, MA is seeking dedicated staff to provide individual and group support in a residential program, helping individuals gain independence and meaningful community participation. You will plan recreational activities, encourage self-advocacy, assist with daily living, monitor safety, and coordinate with nurses and case managers to ensure seamless support.

Driving locally is required to facilitate community access and appointments.

Formación

• High school diploma or equivalent required.
• Valid US driver’s license and safe driving record.
• CPR and First Aid certifications required within specified timeframes.
• MAP and PBS Tier 1 within indicated days.
• Ability to lift at least 25 pounds.

Responsabilidades

• Assist individuals in becoming fully integrated into their communities and develop friendships.
• Access community-based support and resources for individuals.
• Plan recreational and social activities promoting inclusion and well-being.
• Empower individuals to make choices and be self-advocates.
• Provide transportation as necessary.
• Maintain records and progress notes; report changes to supervisor.
• Administer and document medications per MAP requirements.
• Coordinate with nurses, case managers, and Visiting Nurses Association.
• Ensure compliance with safety, policies, and training.
