CenClear is seeking an Independent Living Case Manager to support youth in foster care across Clearfield County as they transition to independent adulthood. You will develop individualized plans, coordinate with community partners, and empower youths with education, job training, and tutoring opportunities.

Ideal candidates show empathy, strong communication, and the ability to maintain boundaries while delivering direct support. Travel within the county is required with mileage reimbursement.

Qualifications

• Empathy and strong communication are essential.
• Able to maintain appropriate boundaries while providing direct support.
• Experience working with youth preferred.

Responsibilities

• Travel within Clearfield County; mileage is reimbursed.
• Work 37.5 hours per week, year-round.
• Build relationships with community partners to connect youth with resources.
• Set and monitor individual goals; conduct assessments and sessions.
• Submit timely documentation and meet program standards.
• Collaborate with the Therapeutic Recreation team and participate in training.
