Job summary

Epworth Children's Home in South Carolina is seeking an Independent Living Case Manager to document services, maintain up-to-date client files, complete intake assessments, and develop care and transition plans for youth in the program.

The role requires strong collaboration on an interdisciplinary team, ability to respond to behavior crises, and coordinating housing, education, health care, and supports to help clients reach self-sufficiency.

Qualifications

• Master's degree in a Human Service field or a Bachelor's degree with 1 year in residential group care.
• Associate's Degree with 2 years in residential child group care.
• Four years of experience at Epworth.
• Prefer at least four years with children and families, three in residential group care.

Responsibilities

• Follow ECH Staff Manual policies and SCDSS/group home regulations.
• Attend supervision meetings and training as scheduled.
• Attend Interdisciplinary Meetings.
• Respond to behavior crises in the center as needed.
• Complete intake and follow-up assessments for all clients.
• Schedule physicals, dental, and vision appointments for clients.
• Ensure clients reach self-sufficiency goals and document progress.
• Assist clients with housing and employment opportunities.
• Ensure access to programs for education, employment, health, and mental health.
• Accompany clients to medical/psychiatric appointments and document procedures.
• Collaborate with interdisciplinary team to achieve client goals.
• Teach life skills through group and individual sessions.
• Hold Review Meetings with placing agency and providers.
