Job summary

Tabor Services in Pennsylvania seeks a Supervised Independent Living Case Manager to provide support, develop individualized life skills plans, and advocate for youth. You will coordinate services, link to community resources, and work with families to promote emancipation and independence.

This role follows a hybrid schedule (3 days in the office, 2 days WFM) with hours 9 a.m.–5 p.m., Monday through Friday, subject to departmental needs.

Qualifications

• Bachelor's degree in Social Work or related field.
• Two years of experience in child, youth, and family service systems.
• Valid driver’s license and ability to use your own vehicle.
• Clean driving history record.
• Must be free from any communicable or infectious diseases.
• Provide PA child abuse clearance, FBI check, and criminal background check.
• Knowledge of PA Code 55 3680s, 3700s, and the Philadelphia DHS scopes of service.

Responsibilities

• Maintains a caseload of independent living youth.
• Contacts with each youth as mandated by SIL standards and regulations.
• Develops and implements an individualized service plan including life skills and discharge planning.
• Coordinates resources and apartment location for service provision.
• Collaborates with families and other professionals to meet youth goals.
• Develops group structure to present life skills to older adolescents.
• Maintains complete client files and mandated reports for regulatory compliance.

Education
