Qualifications

• Education: Bachelor's degree plus two years’ experience providing direct services to individuals with mental disabilities or other relevant human service experience.

Responsibilities

• Support consumers in independent living; provide crisis intervention and counseling.
• Develop and monitor case plans; document progress and services.
• Travel to residents' residences to deliver supportive services.
• Coordinate medical, psychosocial, educational, financial and vocational resources.
• Advocate for residents and maintain accurate case records.
