Job summary

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT) to join our on-site team serving seniors in skilled nursing facilities. The role offers autonomy like private practice, with a steady schedule and a strong emphasis on documentation and facility communication.

You will assess residents, provide psychotherapy, collaborate with an interdisciplinary team, manage your caseload, and stay current with Medicare and commercial insurance plans in a supportive, growth-oriented environment.

Qualifications

• Master’s degree and a valid LMFT/LPC license in the applying state.
• Two years of counseling and therapy experience.
• Ability to work as a team with a great attitude.
• Knowledge of mental health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams including MPAC providers.
• Strong communication with the facility team and Regional Clinical Manager.
• Ability to initiate and communicate across multiple settings.
• Strong organizational skills to manage patient needs and documentation.
• Comfortable with the dynamic environment of a skilled nursing facility.

Responsibilities

• Provide initial assessments and psychotherapy to address mental health issues.
• On-site clinical provider working with residents and families.
• Lead member on interdisciplinary team addressing residents’ needs.
• Manage your caseload, schedule, and collaboration with care teams.
• Document thoroughly and stay updated on CMS/Medicare plans.
