Job summary

MPAC Healthcare is seeking licensed clinicians (LCPC, LMFT, LCSW) to provide on-site clinical care for seniors. You will work with an interdisciplinary team, manage your caseload and schedule, and deliver psychotherapy while handling Medicare and insurance documentation.

Qualifications

• Master’s degree and valid LCSW/LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible.
• The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
• Ability to take initiative and effectively communicate in multiple settings with multiple parties.

Responsibilities

• Provide on-site clinical services to residents and families.
• Lead interdisciplinary team addressing residents’ emotional, social, and psychological needs.
• Manage caseload, schedule, and ultimately, salary with autonomy.
• Provide initial assessments and ongoing psychotherapy.
• Maintain Medicare/insurance documentation and CMS compliance.
