Job summary

American RENOLIT Corporation is seeking a Registered Nurse/Case Manager for Hospice Care in Mishawaka, Indiana. The role runs M-F, 8am to 5pm, with a culture emphasizing excellence, compassion, and trust. Flexible scheduling supports one-on-one patient time.

Responsibilities include comprehensive patient assessments, care planning, pain management, and collaboration with an interdisciplinary team to ensure end-of-life care is patient-centered and respectful of family needs.

Qualifications

• Licensed RN in State of Indiana.
• Current CPR certification.
• Must have and maintain a valid driver's license, good driving record and auto accident insurance.

Responsibilities

• Performs an initial and ongoing comprehensive assessment within 5 days of hospice election and reassessments every 15 days.
• Develops an individualized plan of care in collaboration with patient and family.
• Assesses pain and comfort, implementing a pain management plan.
• Provides patient/family-centered care in the patient’s residence to improve quality of life.
• Collaborates with the interdisciplinary team and other care providers.
• Supervises LPN and Home Health/Hospice Aides per policy and regs.
• Performs admissions and completes ~25 visits per week.
• Documents nursing assessments, goals, care provided, and outcomes.
