Qualifications

• Graduate of an accredited school of nursing with 1–2 years of recent acute care experience.
• Current licensure in state and CPR certification.
• Bachelor's degree preferred, with 1 year home health care experience.
• Excellent observation, verbal and written communication, and problem-solving skills.
• Regular travel by personal vehicle to patient homes and related locations.

Responsibilities

• Assess each new patient's condition, history, and home care needs.
• Build and update the patient care plan with the patient and family.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and provide updates every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange equipment or services the patient needs.
• Check in on home health aide care every two weeks.
