Qualifications

• Degree in physical therapy (Baccalaureate or Master’s) approved by a recognized body.
• Licensed to practice as a physical therapist in Florida, if applicable.
• Two years of physical therapy experience; home health experience preferred.
• Strong verbal/written communication and organizational skills.
• Current CPR certification.
• Must travel between patient homes/locations using personal vehicle with valid driver's license and insurance.

Responsibilities

• Assess patients’ functional status, muscle strength, mobility, gait, ROM, and rehab potential.
• Carry out the physician’s plan of care.
• Instruct patients/families on therapeutic devices and appliances.
• Document care accurately and timely, including progress notes.
• Coordinate with physicians and report treatment responses or changes in condition.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Supervise PT assistants per policy and state regulations.
• Monitor home health aide care every two weeks when therapy is the primary skilled service.
