Qualifications

• Requires degree in physical therapy and state license.
• Two years of PT experience; home health preferred.
• CPR certification and strong communication skills.

Responsibilities

• Assess patients' functional status, mobility, and rehab potential.
• Carry out the physician's plan of care.
• Instruct on prosthetic devices and therapeutic appliances.
• Document care accurately with progress summaries.
• Coordinate with physicians and report treatment responses.
• Identify needs for other home health services and refer.
• Discharge planning and case conferences.
• Supervise PT assistants per policy and state regulations.
• Support families and provide inservice education.
