Qualificações

• Observational skills to assess patient condition and needs.
• Strong verbal and written communication.
• Problem solving and critical thinking in care planning.

Responsabilidades

• Assess each new patient's condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient's status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange equipment or services the patient needs.
• Check in on home health aide care every two weeks.
