Job summary

Advanced Nursing Concepts provides one-on-one home health care to patients in their homes. The PT will assess functional status, implement plans of care, and educate patients and families on devices. Travel to patient homes is required, with a valid license and reliable vehicle.

We offer flexible scheduling, mentorship, and clinical guidance while supporting clinicians with field resources. This PRN/home health role emphasizes independence within the home setting.

Qualifications

• Degree from a baccalaureate or Master’s program in physical therapy.
• Licensed to practice as a physical therapist within the state.
• Two years of appropriate experience as a physical therapist; home health preferred.
• Good verbal and written communication and organization skills.
• Current CPR certification.
• Must travel by personal vehicle between patient homes and other locations; valid driver’s license and reliable transportation.

Responsibilities

• Assess patients’ functional status, muscle strength, mobility, gait, ROM, and rehab potential.
• Carry out the physical therapy plan of care set by the physician.
• Instruct patients and families on therapeutic appliances and prosthetic devices.
• Document care accurately and on time, including progress summaries.
• Coordinate with physicians and report patient response to treatment or changes in condition.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Supervise physical therapy assistants per policy and state regulations.
• Check on home health aide care every two weeks when therapy is the only skilled service.
• Take on other duties as assigned.
