Job summary

Advanced Nursing Concepts provides one-on-one home health care to patients in their homes. The clinician will assess conditions, build and update care plans, administer medications, and teach patients and families to manage care at home. Travel between patient homes and offices is required within the service area.

The role requires RN licensure, CPR, and reliable transportation. This PRN, 2–3 days weekly schedule from 7:00 am to 4:00 pm offers negotiable pay based on experience and abilities.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Preferred: Bachelor's degree, with one year of home health care experience.

Responsibilities

• Assess each new patient's condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient's status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates at least every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange any equipment or services the patient needs.
• Check in on home health aide care every two weeks.
