Job summary

BAYADA Home Health Care in Hudson County, NJ is seeking an experienced Physical Therapist to join our home health visits team. This full-time role provides one-on-one care for adult and geriatric clients across southern Hudson County, with autonomy to manage your schedule while collaborating with a supportive interdisciplinary team.

Responsibilities include skilled visits, evaluations, and individualized plans using evidence-based methods.

Qualifications

• Current New Jersey PT license required.
• Graduation from an accredited PT program.
• Home health experience preferred but not required.
• Strong communication, organizational and assessment skills.

Responsibilities

• Perform skilled PT visits throughout designated territories.
• Complete comprehensive PT evaluations and ongoing reassessments.
• Assess mobility, strength, balance, gait, transfers and endurance.
• Develop and implement individualized treatment plans with evidence-based interventions.
• Provide therapeutic exercise, gait, balance training, manual therapy and education.
• Collaborate with nurses and physicians to coordinate patient care.
• Educate patients and caregivers on exercises, safety and falls prevention.
• Document timely and accurate notes using BAYADA EMR.
