Job summary

BAYADA Home Health Care is seeking a compassionate Occupational Therapist to provide one-on-one home health visits to adults and seniors in Monmouth, Middlesex, Somerset, and Union Counties, NJ.

You will assess patients in their homes, create individualized plans, and collaborate with an interdisciplinary team to promote independence and safety. Compensation includes a $10,000 sign-on bonus for full-time OTs and flexible full-time or per diem opportunities.

Qualifications

• : Current OT license in New Jersey, active and in good standing.
• : Minimum of one year of OT experience.
• : Home Health experience preferred but not required.
• : Graduate of an accredited Occupational Therapy program.
• : Strong clinical assessment and critical thinking skills.
• : Excellent communication, organization, and time management.
• : Comfortable using electronic medical records (EMR).
• : Ability to work independently while collaborating with an interdisciplinary team.
• : Reliable transportation and willingness to travel throughout Monmouth, Middlesex, Somerset, and Union Counties.

Responsibilities

• Provide occupational therapy services to adult and geriatric patients in their homes.
• Complete comprehensive evaluations of patients' functional abilities (ADLs, IADLs, strength, endurance, balance, mobility).
• Develop individualized treatment plans to maximize independence and safety.
• Assess home environments and recommend adaptive equipment and environmental modifications.
• Implement therapeutic interventions to improve function and quality of life.
• Collaborate with nursing and the interdisciplinary care team to coordinate patient care.
• Educate patients and families on techniques, home safety, and independence strategies.
• Reassess progress and modify treatment plans as needed.
• Complete accurate EMR documentation.
