Job summary

BAYADA Home Health Care is seeking a Full-Time Physical Therapist to provide one-on-one home health visits to adult and geriatric clients throughout Burlington County, NJ. You will assess and treat in clients' homes, coordinating with an interdisciplinary team to ensure optimal outcomes.

Qualifications

• Current New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Home health experience preferred, but not required.
• Strong clinical assessment, communication, interpersonal, and organizational skills.
• Ability to work independently and effectively manage time and schedule.
• Prior experience with electronic medical records (EMR) preferred.

Responsibilities

• Provide one-on-one physical therapy visits to clients in their homes throughout Burlington County.
• Perform comprehensive PT evaluations of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Develop and implement individualized plans of care based on client needs and physician orders.
• Provide skilled PT interventions, including therapeutic exercise, gait and balance training, mobility training, and other treatments.
• Monitor client progress and adjust treatment plans as appropriate.
• Educate clients, families, and caregivers on exercises, mobility techniques, fall prevention, safety, and ongoing care.
