Job summary

VitalCaring is seeking a Home Health Physical Therapist to evaluate patients, develop individualized plans, and deliver hands-on therapy in patients' homes. You’ll collaborate with physicians, nurses, and the care team to ensure mobility, safety, and independence.

Ideal candidates have a PT degree, current license, and 1–2 years of home health experience, with strong communication and caregiving skills. Flexible visits and autonomous practice are offered.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately and timely within the EMR.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
