VitalCaring Group is hiring a Home Health Physical Therapist to deliver patient-centered care in patients' homes, helping them regain mobility, independence, and confidence. This highly autonomous, relationship-driven role partners with physicians, nurses, and other clinicians to coordinate care across visits and documentation.

Qualified candidates hold a DPT or Master’s in Physical Therapy, a current PT license in the state, 1–2 years of home health experience preferred, CPR certification, and

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsabilités

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
