FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver high-quality home health care as an independent contractor serving Panorama City and surrounding areas. You will work autonomously with flexible scheduling and receive administrative support to focus on patient care.

Responsibilities include treating patients across ages, documenting progress, and coordinating care using our EMR and Cliniconnects app. A current CA PT license and an accredited PT program are required.

Qualifikationen

• Ability to work with patients of all ages.
• Current Physical Therapist license issued by the state board.
• Completion of an accredited Physical Therapy program.

Aufgaben

• Provide home health PT services to patients in Panorama City and surrounding areas.
• Observe, document, and report patient responses; adjust plan of care.
• Complete documentation using the EMR; participate in care coordination.
