Job summary

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages on a flexible independent contractor basis in Thousand Oaks and surrounding areas.

You control your schedule, work in your preferred coverage areas, and determine earnings by the number of patients treated, with strong per-visit rates. Administrative support is provided so you can focus on treating patients.

Qualifications

• Completion of an accredited Physical Therapy program.
• Current Physical Therapist license issued by the state board.
• License must be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted.
• Enthusiasm and ability to work with patients of all ages.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report patient responses to treatment and changes in condition.
• Determine and implement the plan of care for each patient.
• Use our EMR/documentation system efficiently during visits.
