Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist Assistant to deliver premier care in the Moss Landing area. The role focuses on assessing needs, planning and implementing a treatment program, and monitoring patient progress in home health settings.

You will work as an independent contractor, serving adults of all ages with emphasis on movement, pain reduction, and functional restoration, while maintaining accurate records and collaborating with the broader care team.

Qualifications

• Licensed Physical Therapist with state registration.
• Completion of a PT curriculum approved by recognized bodies.

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, and restore function.
• Determine and implement a treatment plan to improve patient well-being.
• Execute interventions and monitor patient progress.
• Provide Home Health PT services to adults in Moss Landing area.
