Job summary

Qualifications

• Active, unencumbered SC NP license.
• National Board Certification (AANP/ANCC) or equivalent.
• Ability to work independently as 1099 contractor.

Responsibilities

• Manage follow-up visits for patients in PM&R.
• Coordinate with physicians, therapy teams and internal medicine providers.
• Participate in optional Transitional Care Management activities.
• Efficiently document encounters and progress notes.
