Qualifications

• Two years of college education or equivalent experience required.
• Valid driver’s license and driving record meeting company criteria; own transportation recommended.
• Obtain and maintain first aid/CPR and Medication Management certification.
• Experience in recreational or human services preferred.

Responsibilities

• Assess, develop, and implement programming for participants, including job readiness, independent living, education, training and support needs.
• Maintain accurate case files and monitor attendance and progress toward goals with regular updates.
• Provide direct support and develop person-centered Individual Plans with input from participants and stakeholders.
• Participate in interdisciplinary staffing meetings and engage with families and funding sources.
• Help develop community resources to support communication, socialization, daily living skills and community integration.
