Job summary

Goodwill Greater Milwaukee & Chicago seeks a Day Program Specialist to assess, develop, and implement programming for participants, including job readiness, independent living, education, training needs, and support needs. You will create and monitor individualized development plans and ensure program quality.

You will collaborate with interdisciplinary staff, maintain case files, and engage families and funding sources while ensuring CARF and contract compliance and offering

Responsibilities

• Assess, develop, and implement programming for participants, including job readiness, independent living, education, training needs and support needs.
• Develop and monitor individualized development plans with goals for participants.
• Maintain accurate case file records and monitor attendance and progress toward goals.
• Provide direct support with activities based on participant needs; create person-centered plans.
• Attend and participate in interdisciplinary staffings; build relationships with families and funding sources.
• Assist with developing community resources to improve communication, socialization, daily living skills, and community integration.
• Ensure van safety and maintenance; complete annual van training and compliance.
