Job summary

ADAPT Community Network is seeking a Habilitation Assistant to support individuals with developmental disabilities in achieving greater independence through meaningful daily activities and individualized program plans.

Responsibilities include assisting with personal care, documenting progress, and participating in team meetings to enhance service delivery. A valid NYS driver’s license and AMAP certification are required or to be obtained within six months. Competitive benefits are offered.

Responsibilities

• Provide direct support and personal care for daily living activities.
• Assist with individualized program plans and behavior support strategies.

• Supervise individuals in classroom, community, and recreational activities.
• Drive agency vehicles as needed and document progress.
