Job summary

MFM Health is hiring a Triage Nurse (In‑Person) to join our clinical team at Danvers and Middleton locations. This in‑person role focuses on patient assessment, triage, care coordination, and accurate documentation to ensure a seamless patient experience.

The ideal candidate has a current RN or LPN license, 3+ years of clinical nursing experience with triage background, excellent telephone skills, and the ability to work calmly under pressure.

Qualifications

• Current RN or LPN license (as required by state regulations).
• Minimum of 3 years clinical nursing experience; triage experience strongly preferred.
• Excellent telephone communication skills.
• Ability to make sound clinical decisions quickly and accurately.
• Strong organizational and documentation skills.
• Ability to manage stress, multitask, and remain calm in urgent situations.

Responsibilities

• Patient Assessment & Triage: Evaluate symptoms and determine urgency of care.
• Care Coordination: Coordinate follow-up care and referrals.
• Communication: Manage telephone triage and document interactions in the medical record.
• Nurse Visits: Review vaccines and administer vaccines as indicated; perform related procedures.
• Teamwork & Support: Collaborate with departments and mentor peers.
• Professional Conduct: Maintain professionalism and adhere to policies.
