AccentCare Inc. is actively seeking an Admissions Coordinator in Newark, DE. This full-time role focuses on intake, referral management, and coordination of hospice services.

You will collaborate with referral sources, physicians, and the care team to ensure timely admissions and a smooth patient onboarding process. The position requires strong communication and organizational skills, with a preference for candidates who have hospice experience and knowledge of related regulations.

Qualifications

• Associate degree preferred.
• 1+ year hospice experience.
• Excellent written and verbal communication skills.
• Strong organizational skills.
• Excellent customer service background.
• Familiarity with hospice policies and regulations.

Responsibilities

• Receive professional referrals and obtain referral information for timely response and data entry in the EMR system.
• Handle inquiries regarding Hospice services and input referrals into EMR with basic patient status data.
• Coordinate and schedule visits ensuring staff contact based on patient condition and family situation.
• Communicate with referral sources, physicians, Seasons staff, and other agencies to initiate hospice services.
• Maintain regular communication through sign-on, consent and admission assessment phases for seamless coordination.
• Maintain accurate EMR documentation and referral statistics.
