Job summary

AccentCare is hiring a Hospice Aide Coordinator in Newark, with a M-F schedule and a salary of $21.00 - $23.00 per hour. This full-time, on-site role requires a Nurse Aide certification and at least two years in hospice care, plus strong Microsoft Office and EMR skills.

You will coordinate HA schedules, mentor staff, manage supplies, and provide patient care as needed. The position emphasizes collaboration with the interdisciplinary team, families, and facilities to ensure compassionate,

Qualifications

• High school diploma required, Nurse Aide certification in CHHA/CGNA preferred.
• CPR certification preferred.
• At least 2 years of hospice care experience; CHAPNA preferred.
• Proficiency in Microsoft Office and EMR with strong communication.
• Must have reliable transportation, valid driver’s license, and physical stamina.

Responsibilities

• Coordinate and assign HA schedules and visits with nursing leadership.
• Mentor HAs, support training, competency reviews, and QAPI participation.
• Oversee medical supply management including ordering and cost control.
• Provide direct patient care as needed per the care plan and document.
• Collaborate with interdisciplinary team, facilities, and families for coordinated care.
• Take on additional duties as assigned, possibly carrying a patient caseload.
