Job summary

Bear Mountain Healthcare is seeking an Occupational Therapist to join our In House Rehab Team. This position supports Speech Therapy services within our skilled nursing facility, providing plan development, program evaluation and coordination with the care team.

Candidates must hold an active state license or be license eligible, have an M.A./M.S. in Speech-Language Pathology, CCC or CFY status, and be eligible for licensure.

Qualifications

• Active state license or license eligible (as defined for that license type).
• M.A./M.S. in Speech and Language Pathology from an accredited college.
• CCC or CFY status and eligible for licensure.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct Speech therapy services as well as its programs in accordance with regulations governing the facility.
• Maintains and completes accurate documentation, department records and reports.
• Initiates referrals when appropriate and in accordance with Department procedures.
• Conducts screenings of residents to determine need for intervention/treatment and communicates findings.
