Evolve Therapy Services in Freeport, IL is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. You will provide quality speech therapy services, evaluate data for treatment planning, and ensure CMS-compliant documentation.

This role emphasizes collaboration with patient families and staff, FEES/MBS protocols oversight, and ongoing professional development to maintain licensure.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, per standards and workflow.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with target lists, short- and long-term goals.
• Instruct patients, families and staff to carry over therapy goals in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify residents' needs with clinical excellence.
