Job summary

Evolve Therapy Services in Freeport, IL seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. You will provide quality therapy services, plan and implement treatment, and collaborate with staff to ensure high-quality resident care.

Responsibilities include evaluating data, developing treatment plans with goals, documenting in NetHealth EMR, and coordinating FEES/MBS protocols.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with target, short-term, and long-term goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to identify needs and deliver timely care.
