Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Freeport, IL. This PRN position focuses on planning, directing SLP services to ensure high-quality resident care.

The role requires a Master's in Speech Language Pathology, CCC, and an active IL SLP license, with ongoing CE and professional development to maintain licensure and certification. Proficiency with NetHealth EMR is expected.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with short-term and long-term goals.
• Instruct patient, family, and staff to ensure carryover of goals.
• Complete required CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in screening protocols to meet resident needs.
