Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to work in-house at a Skilled Nursing Facility in Freeport, IL. This PRN role focuses on providing high-quality speech therapy, evaluating residents, and developing treatment plans to improve communication and swallowing outcomes.

The position requires an active SLP license and CCC, with CMS documentation responsibilities and coordination of FEES/MBS protocols.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency required.
• Active SLP license in practicing states.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to ensure needs are met timely and with clinical excellence.
