Job summary

Evolve Therapy Services, LLC is seeking a Speech-Language Pathologist for an in-house Skilled Nursing Facility in Freeport, IL. The role focuses on planning, directing, and delivering Speech Language Pathology services in compliance with standards and regulations.

The position emphasizes evaluation, treatment planning, staff and family instruction, and CMS-compliant documentation through NetHealth EMR, with opportunities for professional development and internal growth.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC certification preferred/required.
• Active SLP license in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families, and staff to carry over therapy goals in daily activities.
• Complete compliant CMS documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify resident needs timely.
