Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Freeport, IL. This PRN role supports planning, developing, and directing SLP services to ensure high-quality resident care.

You will evaluate data for treatment planning, develop goals, educate patients and staff, and ensure CMS documentation in NetHealth is compliant. Experience with FEES/MBS coordination is valued.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant CMS documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to establish needs with clinical excellence.
