Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Oconto, WI. The PRN position supports resident therapy plans under a Physical Therapist, performing screenings, treatments, and education.

Qualifications include an Associate's degree in physical therapy and an active PTA license; ongoing continuing education to maintain licensure is expected. Competitive benefits apply based on status.

• Associate degree in physical therapy from an accredited program.
• Must have an active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Mansioni

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation with the Physical Therapist.
• Assist in developing target list with short- and long-term goals and treatment plan.
• Conduct individual and group treatment addressing strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting techniques/media and adapt as indicated.
• Monitor residents’ response and modify as needed with PT input.
• Assist in establishing resident schedules with the PT and team members.
• Train residents, family and staff in equipment use.
• Participate in resident, family, and staff education.
• Ensure documentation per CMS expectations in NetHealth EMR.
