Job summary

Creative Solutions In HealthCare is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care. The SLP will assess swallowing and cognitive function, design individualized treatment plans, and provide AAC support as needed.

The role emphasizes collaboration with care staff, education for residents and families, and thorough documentation. Texas licensure and LTC experience are preferred; in-house benefits offered.

Qualifications

• Active Texas license in Speech-Language Pathology is required.
• Strong communication skills and empathy for residents are essential.
• Ability to adapt to residents with varying personalities and needs.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary assessments and implement individualized treatment plans.
• Develop and implement AAC systems for complex communication disorders.
• Provide training to clients, families, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate competent technical skills per speech therapy standards of practice.
• Communicate changes in residents' condition and document billing and records.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other staff.
• Ensure resident safety and confidentiality; comply with laws.
