Job summary

Five Points Nursing and Rehabilitation is seeking a skilled Speech-Language Pathologist to join our in-house team serving residents with person-centered care. The role involves evaluating swallowing and cognitive function, developing individualized treatment plans, and educating residents and families.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology
• In good standing with regulatory agencies and licensing boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate residents' swallowing or cognitive conditions.
• Help residents achieve goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop treatment plans.
• Provide AAC systems for severe expressive/language disorders.
• Coordinate with other professionals for diagnostic swallowing procedures.
