Job summary

Creative Solutions in Healthcare, Inc. is seeking a Speech-Language Pathologist (SLP) to join our in-house team serving residents with person-centered care to achieve positive outcomes.

The role includes evaluating swallowing and cognitive deficits, developing individualized treatment plans, and providing education to residents and caregivers. The position offers full-time and PRN options with comprehensive benefits, including health/dental/vision insurance, life insurance, tuition

Qualifications

• Active Texas license in Speech-Language Pathology.
• SNF/Long term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents’ swallowing or cognitive deficits.
• Help residents improve cognition or swallowing goals.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for severe expressive and/or language disorders.
• Train clients, families, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate competent technical skills per current speech therapy standards.
• Communicate changes in resident’s condition.
• Provide billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other staff.
• Ensure no abuse or neglect of residents.
• Observe confidentiality of patient information.
• Comply with laws and professional requirements.
