Job summary

Evolve Therapy Services in Kingsport, TN is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will provide physical therapy services under the supervision of the Physical Therapist and participate in resident screenings and treatment planning.

Responsibilities include conducting individual and group therapies, documenting progress in NetHealth EMR, and coordinating with the care team for efficient schedules and quality care.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Experience with CMS documentation preferred.

Responsibilities

• Provide physical therapy services under direct supervision of the Physical Therapist.
• Screen residents regularly to determine intervention needs.
• Develop and modify treatment plans with the Physical Therapist.
• Conduct both individual and group treatment focusing on mobility, gait, strength, and balance.
• Document sessions in NetHealth EMR per CMS requirements.
• Coordinate schedules with the care team.
