Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Dyersburg, TN. This PRN position supports the Physical Therapist with resident care, evaluations, treatment planning, and routine CPT-based therapies.

You will assist in scheduled treatments, participate in CMS-compliant documentation in NetHealth EMR, and help educate residents and families. The role requires an active PTA license and an Associate's degree.

Qualifikationen

• Active PTA license is required in state(s) of practice.
• Continuing education and professional development are expected.
• Ongoing licensure and certification must be maintained.

Aufgaben

• Provide physical therapy services to residents under direction of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and treatment planning under the Physical Therapist's direction.
• Develop target goals and treatment plans with the PT.
• Deliver individual and group treatment per the plan, focusing on mobility and strength.
• Assist in documentation and adherence to CMS standards in NetHealth EMR.
