Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Hadley, MA. Full-time, part-time, or PRN positions available with a $3,000 sign-on bonus.

You will plan, direct, and deliver PT services to residents, ensuring quality care and compliance with all regulations. Responsibilities include evaluating residents, developing and implementing treatment plans, documenting per CMS, and collaborating with families and the interdisciplinary team.

Formación

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsabilidades

• Provide physical therapy services to residents in accordance with standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention.
• Screen residents referred to PT to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and methods.
• Communicate evaluation findings and plans to residents, families, and team.
• Complete compliant documentation per CMS expectations in NetHealth EMR.
• Perform other duties as assigned to support resident care.
