Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Antioch, TN. The role supports residents with PT care, evaluations, and treatment planning in a CMS-compliant environment.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Develop and implement treatment plans with short- and long-term goals.
• Screen residents regularly to determine intervention needs.
• Communicate evaluation findings and recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
