Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapy Assistant for our in-house Skilled Nursing Facility in Bradford, PA. You will provide physical therapy services under the supervision of a Physical Therapist, conduct screenings, assist in evaluations, and help implement treatment plans to support residents’ mobility and quality of life.

Requirements include an Associate’s degree in physical therapy, an active PTA license in the state(s) of practice, and ongoing continuing

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in the state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and goal setting under the Physical Therapist.
• Assist in treatment plan development and implementation.
• Educate residents, families and staff on therapy activities.
• Document CMS-required notes in NetHealth EMR.
