Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Bradford, PA. This full-time or PRN position supports planning, organizing, and directing physical therapy services to ensure high-quality resident care.

You will evaluate residents, develop treatment plans with short- and long-term goals, and document in NetHealth EMR. Active licensure and ongoing CE are required, with competitive pay and benefits for full-time staff.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct regular screenings to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Complete CMS-compliant documentation from NetHealth EMR systems and other duties as assigned.
