Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Bradford, PA. This full-time or PRN role focuses on planning, organizing, and directing physical therapy services to ensure high-quality resident care in compliance with state and federal standards.

Responsibilities include evaluation, treatment planning, documentation, and collaboration with families and an interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards of practice and department procedures.
• Document and bill therapies in accordance with CMS/NetHealth requirements.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Screen residents regularly to determine need for intervention or treatment.
• Communicate findings and treatment plans to residents, families, and the interdisciplinary team.
• Ensure compliant and medically necessary documentation is completed.
• Perform other duties as assigned to support resident care and facility needs.
