Job summary

Evolve Therapy Services, LLC is seeking a focused Physical Therapy Assistant to provide in-house skilled nursing facility therapy under a PT's supervision. The role covers screenings, evaluations support, goal setting, and implementation of therapy plans.

You will work with a multidisciplinary team to deliver hands-on treatment, monitor progress, and ensure compliant documentation in NetHealth EMR. A relocation or sign-on bonus is available for the right candidate.

Qualifications

• Associate's degree in physical therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and in identifying goals and treatment plans.
• Carry out individual and group treatments including strength, mobility, gait, balance, and ROM activities.
• Assist in selecting techniques/media and adjust activities per resident performance.
• Monitor residents' response and consult with the Physical Therapist.
• Maintain resident schedules and coordinate with the care team.
• Train residents and staff in the use of equipment and participate in education.
• Ensure timely and accurate documentation in NetHealth EMR per CMS expectations.
