Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role involves working under the supervision of a Physical Therapist to deliver therapy services to residents.

Qualifications

• Associate's degree in Physical Therapy required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and in developing treatment plans under supervision.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist.
