Job summary

Evolve Therapy Services is seeking compassionate Physical Therapists for an in-house Skilled Nursing Facility in Olivia, MN. Full-time, part-time, and PRN positions are available with a $5,000 sign-on and tuition reimbursement.

Responsibilities include planning, directing PT services, documenting and billing, and developing treatment plans to improve resident outcomes. Licensure and ongoing CEU compliance are required.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
• Must have an active Physical Therapy license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures
• Adhere to documentation and billing procedures for physical therapy services
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs
• Conduct screening of residents to determine need for intervention/treatment
• Develop treatment plans with short- and long-term goals and methods to achieve them
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families/teams
• Complete CMS-compliant documentation in NetHealth EMR systems
